    End of life decisions
    That was a very thoughtful show. I’m 72 and think about dying everyday now that the virus is here. But the actual dying is the least of my concerns. Getting one’s life in order, even down to who will care for my cat, is an enormous undertaking. This woman knows she is dying and has the means to do so, but she clings to life like all of us do. I’m in perpetual quarantine and that is killing me too.

    ? Error in this week’s podcast
    I have really enjoyed this podcast but I think I heard an error by one of the guests in this week’s episode. Syphilis is caused by a bacterium not a virus! The guest was talking about the horrific Tuskegee Study and a similar one in Guatemala. She kept referring to viruses in her comments. I was surprised that Dr Emanuel did not clarify the issue. Confusing viruses and bacteria contributes to inappropriate antibiotic use.

    They Know The Score!
    Sounds wonky and depressing, but it really isn't. If anything. these pros quietly assure me that all this madness will inevitably flow toward a known solution. It may take a very long time if people don't hang tough, but these guys just patiently forge ahead with the wisdom of people who know the score.

    Superb. Thoughtful, intelligent, informed discussion by two genuine experts with a gift for plain English. They are talking publicly about tough issues raised by the pandemic, issues that all of us--not just health care providers--need to be thinking about and discussing among ourselves. They did it with the right balance of story-telling by a family member of a patient, an interview with a health care provider and discussion between the two of them. Excellent. Looking forward to future episodes.

    Too Dismissive of disability advocates?
    I really enjoyed the conversation in this podcast and how it made tough ethical questions interesting and accessible. I did feel that perhaps the speakers were a bit too dismissive of the concerns raised by disability rights advocates in times of triage. They said a few times, no one is considering mental disabilities or only conditions impacting prognosis are considered but that’s not the reality on the ground and that should be acknowledged! Alabama adopted a triage plan that did systematically exclude people with intellectual disabilities from ventilator use. NYS’s protocol stated that a patient’s home ventilator could be taken if they arrived at the hospital for non-COVID treatment. Some states restricted access from anyone who premorbidly requires assistance with ADL. When able bodied people are making these decisions they often write policy for QALYs instead of just years of life. The presumption that life as a disabled person is inherently of less value is already steeped throughout our literature. The life years saved of a paralyzed woman are seen as less valuable than those of a woman without paralysis. These kinds of discrimination are prolific and shouldn’t be minimized when discussing the situation at hand or the history of triage scenarios.

    Waste of time
    Podcast a waste of time. So the first episode is all about the shortage of ventilators yet the real life case they use, the old guy died due to his underlying health conditions NOT because he didn’t get a ventilator. Typical fear mongering to force a political view.
